Appointment Info & Release Forms

REDTREE ARTIST & CLIENT RELEASE FORMS:

We hope this finds you well! We are excited for you to get tattooed here at RedTree Tattoo Gallery where we have switched things up to comply with the new Ohio regulations and guidelines for tattoo studios in regards to the Covid-19 Pandemic and opening the State of Ohio back up. Below, we have added new regulations to our already personal strict studio code of cleanliness and sterility here at RedTree Tattoo Gallery!

First, we will need you to be logged into your email account. We then need you to fill out the NEW ONLINE "Client Release Form" at the link below before you arrive at the studio. This is mandatory before your appointment as this minimizes clients and artists touching the same paperwork, pens and other items within the studio. RedTree Tattoo Gallery will need you to fill out the Online release form before we can proceed with your appointment to make the process goes smooth for everyone at RedTree Tattoo Gallery.

REDTREE NEW CLIENT COVID-19  SAFETY PROTOCOLS:

  • DO NOT BRING YOUR CHILDREN TO YOUR APPOINTMENT!
  • If you have flu symptoms, please reschedule your tattoo appointment.
  • Make sure all release forms are completed before arriving.
  • Clients must wear masks at all times before entering the doors.
  • Client must provide their own mask!
  • Clients will be asked to wipe down hands upon entry. 
  • Clients will be asked to sanitize hands upon entry.
  • Clients must provide their own hand sanitizer.
  • Clients will be asked to take quick temperature upon entry.
  • Clients can NOT have company or visitors during consultations or appts.
  • Clients may only bring in essential items such as phones, glasses, etc.
  • Clients are required to sanitize their phones, personal items upon entry.
  • Clients will make artists aware of compromised immune system.
  • Clients must wait outside of studio or in car until their appointment.

REDTREE NEW SAFETY PROCEDURES & PRACTICES:

  • DO NOT BRING YOUR CHILDREN TO YOUR APPOINTMENT!
  • New hand wiping and sanitation areas upon entry.
  • New PPEs worn by all artists.
  • Artist & Client ONLY during all procedures.
  • Hand sanitation areas in every tattoo booth.
  • Disposable covers on all digital equipment.
  • Digital Release Forms filled out prior to appointments.
  • No Contact digital photos portfolios added to the studio.
  • Separated Client and Artist Restrooms and wash rooms.
  • Studio Air Filtration System has been upgraded to Merv13 filters.
  • RedTree Artists & Crew are Covid-19 Airborne Pathogens Certified.
